Download: role of the liver in the toxicology of paracetamol. This review has been written to examine the role of the liver in the toxicity of paracetamol. The possibility of an idiosyncratic sensitivity to this drug has been evaluated and that specific therapeutic measures can reduce the hepatic and systemic toxicity of paracetamol has been tested in animal models. All three stages of paracetamol metabolism have been examined with regard to their involvement in the mechanism of hepatotoxicity. The mechanism of drug-induced cholestasis is considered in detail. The toxicity of paracetamol is thought to be largely due to the oxidative metabolism of this drug to N-acetyl-p-benzoquinone imine (NAPQI). The formation of NAPQI in the liver is not essential for the toxicity of paracetamol. Chloroform, the main metabolite of paracetamol, does not cause any toxicity in itself and is incapable of directly inducing DNA strand breaks. Oxidative stress does not play any important role in the toxicity of paracetamol. In general, the toxic effects of paracetamol can be ascribed to its sulphydryl-reactive metabolites. A reduction in the extent of oxidative stress may help to reduce the toxicity of paracetamol and this may occur either by increasing the capacity of the liver to detoxify reactive metabolites or by inhibiting the formation of reactive metabolites by the cytochrome P450 system.
